Care Coordinator

REIVA is hiring a Care Coordinator for a growing U.S.-based telehealth mental health company. This is a full-time, on-site opportunity in Islamabad for someone with excellent communication skills, a warm and empathetic phone presence, and the ability to confidently support U.S. patients throughout their care journey.

This role supports a growing U.S.-based telehealth mental health company focused on connecting patients with the right care and helping them remain engaged throughout their treatment journey.

The care coordination team plays an important role in the patient experience by communicating directly with patients, helping them connect with therapists, coordinating appointments and following up to make sure they continue receiving the support they need.

About the Role

We are looking for a confident, empathetic and highly organized Care Coordinator who can communicate naturally with U.S. mental health patients and provide a professional, supportive experience throughout the care process.

You will work directly with patients through inbound and outbound communication, help match them with therapists, coordinate appointments, activate new patients and follow up with individuals who need additional support.

A major part of this role involves building trust through conversation. You should be comfortable speaking with a high volume of patients while maintaining warmth, professionalism and attention to detail.

You will also help re-engage patients who have dropped off, maintain accurate patient records and ensure follow-ups are completed consistently.

Key Responsibilities

  • Speak directly with U.S. mental health patients in a warm, professional and empathetic manner.

  • Handle inbound calls and a high volume of outbound patient communication.

  • Match patients with therapists and coordinate appointment scheduling.

  • Support new patient activation and help patients get started with their care.

  • Follow up with patients regarding appointments, next steps and ongoing engagement.

  • Re-engage and reactivate patients who have dropped off or stopped responding.

  • Maintain accurate patient records, communication notes and follow-up information.

  • Keep patient communication organized and ensure required follow-ups are completed.

  • Provide consistent and professional support throughout the patient journey.

  • Work closely with the broader care team to help create a smooth patient experience.

Qualifications

  • An American or near-native neutral accent is required.

  • Strong spoken English and confident verbal communication skills.

  • Warm, empathetic and energetic phone presence.

  • Strong organizational skills and reliable follow-through.

  • Comfortable handling a high volume of patient calls and conversations.

  • Ability to communicate professionally with patients in a mental healthcare setting.

  • Strong attention to detail when managing patient records and follow-ups.

  • Ability to manage multiple conversations, appointments and patient needs effectively.

  • U.S. healthcare or telehealth experience is preferred.

Compensation and Work Arrangement

This is a full-time, on-site position in Soan Gardens, Islamabad, working a night shift from 7 PM to 4 AM.

The advertised salary range is PKR 100,000–150,000 per month.

The role also includes fuel allowance, phone allowance and gym allowance.
